Criteria for Provider Award Nomination
Criteria for Award Nomination:
MNHPC invites the community to recognize outstanding individuals, teams, or organizations who embody the values and mission of hospice care. To ensure that we honor the most deserving individuals, nominees must meet the following criteria:

Member Status: The nominee must be an active member of MNHPC either through individual membership or an organization in which they are directly working for.

Patient Interaction: The nominee must work directly with patients residing in Minnesota, though they may also serve in various other capacities that support hospice care

Exceptional Service: The nominee has demonstrated outstanding service to hospice and/or palliative care, making a significant impact through their dedication.
Timeline
To participate in the nomination and selection process for the MNHPC Volunteer Awards, please adhere to the following timeline:

October 1st, 2025 - January 15th, 2026: Nominations are open. During this period, submit your nominations via the MNHPC website.

January 16 - January 30, 2026: Voting is open to the public. Cast your votes through the conference website to help choose the most deserving provider.

February 2nd, 2026: Winners will be notified. Celebrate the announcement of our award recipients and join us in honoring their exceptional contributions.
