Who should attend?

The Executive Roundtable is designed for leaders of hospice and palliative care across Minnesota, including CEOs, directors, managers, and board members. For November’s agenda, we also encourage participation from nurses, social workers, and professionals in quality and compliance—especially those involved with skilled nursing facilities and the implementation of the HOPE tool.

Annette Lee built Provider Insights to give home health and hospice providers peace of mind. She’s had decades of experience at CMS and provided many years of consultancy services—including contracting as a Medicare Education Consultant for Home Health and Hospice with the MAC.

 

She speaks Medicare-ese fluently and knows how to make the most complicated rules make sense for patient-centered providers who don’t have the time to dissect every rule.

 

Participants will have the opportunity to submit questions ahead of time so Annetee to address. Registered Executive Roundtable participants will also be given access to Annette’s workbook, Hope Toolkit.

Amanda Johnson, RN, LNHA, LALD, CHC, Chief Operating Officer, Care Providers of Minnesota

Ms. Johnson bring over three decades of experience in long term care, having worked She holds a bachelor's degree in human services, is a Registered Nurse, a licensed Nursing Home Administrator, an Assisted Living Administrator and holds a Certification in Health Care Compliance. Throughout her career, Ms. Johnson has held a variety of senior clinical and operational leadership roles in post- acute care settings. Her responsibilities have included developing skilled nursing, assisted living and home care policy and procedures, leading health information technology initiatives, overseeing regulatory and corporate compliance, spearheading collaborative performance improvement projects, and driving clinical, operational, and quality improvement practices. Prior to joining Care Providers of Minnesota, Ms. Johnson served in executive roles for more than 20 years,

My passion for delivering quality health care has guided my role as a nurse for 31 years (25 of these years in hospice), in addition to geriatric nursing, nursing education, and leadership. I have presented both locally and nationally, served as a surveyor for NLNAC, and have been very involved in curriculum development at the college and state level. I currently serve on the Practice & Standards Committee for MNHPC.

Stacy Lysen, President/CEO, Grace Hospice

Stacy Lysen is the CEO of Grace Hospice, bringing over 20 years of experience in the healthcare field with a focus on end-of-life care. A graduate with a Bachelor’s degree in Nursing, Stacy began her career as an oncology nurse, where she developed a deep passion for providing compassionate care to patients facing serious illness. This passion led to a transition into hospice care, where Stacy found her true calling in supporting patients and families during one of life’s most challenging journeys.

After gaining valuable experience in hospice case management, Stacy quickly rose through the leadership ranks, thanks to a combination of clinical expertise and a strong commitment to quality care. As the Director of Grace Hospice during its startup phase, Stacy was instrumental in shaping the organization’s clinical practices and care model, helping to establish Grace as a trusted provider of hospice services in the community.

Promoted to CEO 2 years ago, Stacy now leads Grace Hospice with a vision focused on innovative, patient-centered care and continued growth in meeting the evolving needs of those at the end of life. With a heart for service and a strong background in nursing, Stacy remains dedicated to fostering a compassionate, professional environment for both patients and staff.

Lores Vlaminck, MA, BSN, RN, CHPN

Lores is the founder and principal of Lores Consulting, LLC. Lores has been providing consulting, training, and mentoring for hospice, assisted living, home health care and palliative care organizations for the past 20 years. Currently, Lores is concentrating on the areas of hospice and palliative care.

Lores draws on her 48 years of professional nursing experience to empower client agencies and facility providers to reach their greatest potential. Her extensive nursing background includes clinical practice in cardiac and intensive care, outpatient clinical services as well as serving as a founder/director of a home care and hospice agency for 19 years in greater Minnesota.

In addition, Lores is an approved trainer for the ELNEC (End of Life Nursing Education Curriculum). Lores serves as an active member on one large MN non-profit board.

Lores obtained both her BSN and Master’s degrees from Bethel University, St. Paul, MN

Jessica Sweazey, RN, Director of Nursing, Elim Wellspring

Jessi Sweazey, RN, is the Director of Nursing at Elim Wellspring, a Cassia affiliate skilled nursing facility known for its commitment to high-quality, resident-centered care. With over 20 years of experience in healthcare—spanning both skilled nursing and acute care—Jessi brings deep clinical expertise and a strong focus on team leadership, staff development, and operational excellence.

Jessi is known for her hands-on leadership style, fostering a positive teaching and learning environment where nurses feel supported, empowered, and valued. She believes in a team-based approach to care, encouraging collaboration across all departments to ensure the highest standards of service and compassion for residents and their families.

Throughout her career, Jessi has been dedicated to building strong, cohesive nursing teams, improving clinical outcomes, and cultivating a culture of respect, accountability, and continuous improvement.

Angela Stevens, RN, MBA, LALD, Sr. Director - Clinical Operations Ecumen

Angela provides professional clinical and quality to support to Ecumen communities as the Sr. Director- Clinical Operations.  Angela's nursing leadership career started as the Clinical Director for the Ecumen Pathstone Assisted Living and then transitioned into the Director of Nursing in the same community.  She supported the campus communities in a regional role prior to accepting her current role.  Angela holds a Bachelor of Science in Nursing and a Masters in Business Administration, Healthcare Management.  She is a board member for MNDONA and Center for Nursing Equity and Excellence.

Jessica Hausauer, PhD is CEO of the National Coalition for Hospice and Palliative Care. In this role, Jessica works with members to advance national policy priorities in serious illness care across the lifespan and to advance the Clinical Practice Guidelines for Quality Palliative Care. Jessica represents the Coalition on the National Academies of Sciences, Engineering, and Medicine’s Roundtable on Quality Care for People with Serious Illness, the Patient Quality of Life Coalition, and the Center for Excellence in Assisted Living at UNC. Prior to joining the Coalition, Jessica served as Executive Director of Minnesota Network of Hospice and Palliative Care. From 2020 to 2021, she co-convened the Minnesota Serious Illness Action Network to develop recommendations specific to serious illness care in the pandemic, palliative care policy and payment, and family caregiving. As chair of the Minnesota Palliative Care Advisory Council, Jessica leads efforts to assess access to palliative care across the state, analyze barriers, and make recommendations to the legislature. Jessica earned her PhD in Sociology from Syracuse University Maxwell School of Citizenship and Public Affairs. Jessica will provide federal and state policy updates for palliative care and hospice.
