About the Role
This is a full-time role (32 hours per week) for a social worker at Prairie Grace Hospice. As a Hospice Social Worker, you will be responsible for providing support and counseling to hospice patients and their families, conducting assessments, developing care plans, and maintaining clinical records. You will collaborate with interdisciplinary teams to ensure the emotional and social well-being of patients, and advocate for their needs. Additionally, will serve as a liaison between patients, families and community agencies as well as assist other team members in understanding significant social and emotional factors. This role requires empathy, strong communication skills, and the ability to work effectively in a compassionate and supportive environment.
This role is based in Mankato, MN, with on-call availability needed. Travel is required to Greater Minnesota locations. Willingness to abide by health and safety standards with strong ethical standards is required.
